Coordinated Career Pathways (CCP) 

The Coordinated Career Pathways (CCP) program is a new service at Self Determined Futures LLC that supports individuals with developmental disabilities transitioning from work activity programs or secondary education into competitive integrated employment (CIE). The program features two key services: the Career Pathway Navigator (CPN) and the Customized Employment Specialist (CES). Over 18 to 24 months, participants receive personalized career planning, exploration, training, and job placement support. The CPN helps develop and implement a Person-Centered Career Plan (PCCP), while the CES focuses on securing customized employment opportunities. SDF’s CCP program aligns with our mission to empower marginalized populations, promote social justice, and foster self-sufficiency. ​ Available languages: English, Spanish, ASL

Housing Access
Services (HAS)

At Self Determined Futures LLC, we understand the importance of securing affordable housing and providing essential support for adults seeking independent living. Our Housing Access Services program offers a comprehensive and personalized approach to help clients achieve and maintain housing independence. Our Individual Housing Transition Services help individuals prepare for and move into their own homes through personalized tenant screening, housing assessments, custom support plans, application assistance, utility setup coordination, safety issue resolution, and move-in logistics management. Once housing is secured, our Individual Housing & Tenancy Sustaining Services ensure long-term success and community integration by addressing behaviors that could jeopardize housing, educating tenants and landlords, fostering positive landlord relationships, resolving disputes, preventing eviction, assisting with housing recertification, and providing ongoing tenant training on household management and lease compliance. Independent Living
Skills (ILS)

Our ILS program empowers adults with intellectual or developmental disabilities to enhance their independence through tailored training and support, whether they seek to live alone or with others, autonomy, encompassing a wide range of services from advocacy and self-advocacy to daily living skills, community integration, and safety measures. Our ILS program is customized to accommodate each individual's cultural background, and preferences. Available languages: English, Spanish, ASL

Coordinated Family
Support (CFS)

The CFS program provides essential support to adults with intellectual or developmental disabilities living with their families, assisting with various aspects such as finding medical professionals, scheduling appointments, navigating government programs, and personalized service plans to address individual challenges and goals, all delivered with cultural sensitivity and adaptability. ​ Available languages: English, Spanish, ASL

Supported Living
Services (SLS)

Our SLS program provides comprehensive support for persons with developmental disabilities. The primary focus of our Supportive Living Services is to collaborate with individuals that want to design richer, more connected lives in the home & community of their choosing. Our Supportive Living Services create vision by facilitating person centered planning. Individuals develop creative support teams & schedules to best meet their unique needs. Service and skill development ranges from acquiring housing, safety practices, parenting skills, health maintenance, medication management, household care and maintenance, social skills and relationship building, budgeting, bill paying and financial management, to meal planning, shopping, and food preparation. Self Directed Support (SDS) & Independent Facilitator

With our caring and professional staff we provide comprehensive support, assisting individuals in making informed budget decisions, accessing services according to their individual program plan, addressing immediate and long-term needs, participating in person-centered planning, and acquiring necessary services and supports. Available languages: English, Spanish, ASL

Social Recreation Services (SRS)

Our Social Recreation Program (Service Code 525) is designed to help adults (18 years and older) with developmental disabilities build social skills, gain independence, and get more involved in their communities through fun, meaningful activities. We know how important it is to feel connected, included, and supported, so this program helps individuals explore their interests, make friends, and develop self-advocacy skills in a welcoming community-based environment. Whether it’s volunteering, going on outings, or participating in social or recreational events, everything we do is centered around each person’s goals and what they enjoy. We tailor our support to meet individual needs and focus on outcomes that truly make a difference. Social Recreation is available in Riverside and in Harbor City.
